Offer: Get a $5 rebate on the purchase of two or more boxes of Acuvue Oasys or Acuvue Vita contact lenses.
Eligibility: The offer was open to residents of the United States, Puerto Rico, and the U.S. Virgin Islands.
How to redeem:
- Purchase two or more boxes of Acuvue Oasys or Acuvue Vita contact lenses between January 1, 2016, and December 31, 2016.
- Keep your receipt and the contact lens packaging.
- Complete the rebate form, which was available on the Acuvue website or on the packaging of the contact lenses.
- Mail the completed rebate form, along with your receipt and contact lens packaging, to the address listed on the rebate form.
Rebate amount: $5 per rebate submission, up to a maximum of $10 per household.
Expiration date: The rebate offer expired on December 31, 2016.
