Can i mail my completed driver medical evaluation ds 326
According to the Federal Motor Carrier Safety Administration (FMCSA), the completed Medical Examiner's Certificate (Form MCSA-5876) should be returned to the Medical Examiner (ME) who performed the physical examination, not mailed to the FMCSA.
The ME will then forward the completed certificate to the FMCSA, along with any supporting documentation, as part of the driver's medical certification process.
