Form DS-82: Application for a U.S. Passport by Mail

Important: Make sure to fill out the form accurately and completely. Incomplete or inaccurate forms may cause delays in the processing of your application.

Form Requirements:

  1. Completed Form DS-82: Fill out the form online and print it, or print a blank form and fill it out by hand. Do not sign the form until instructed to do so by an acceptance agent.
  2. Proof of U.S. Citizenship: Provide one of the following:
    • Birth certificate (issued by the city, county, or state)
    • Naturalization certificate
    • Prior U.S. passport
  3. Proof of Identity: Provide one of the following:
    • Valid driver's license
    • Government-issued ID
    • Current military ID
  4. Photocopy of Both Sides of Your ID: Make a photocopy of both sides of your ID and include it with your application.
  5. Passport Photo: Provide one recent, color photograph that meets the Department of State's requirements.
  6. Payment: Pay the application fee with a check or money order made payable to the U.S. Department of State. The current fee for a U.S. passport book is $110 for adults (16 and older) and $80 for children (15 and younger).
  7. Self-Addressed Stamped Envelope: Include a self-addressed, stamped envelope with your application so that the Department of State can return your new passport to you.

Instructions:

  2. Sign the form in front of an acceptance agent or a notary public.
  3. Attach the required documents and payment to the form.
  4. Mail the application to the address listed on the form.
