The SSA-44 form is a Request for Reporting of Changes Form, and it's used to report changes to your Social Security benefits. Here's where you should mail it:

You can mail the completed SSA-44 form to the address listed below, depending on your state:
- If you live in Alabama, Arkansas, Florida, Georgia, Kentucky, Louisiana, Mississippi, North Carolina, Oklahoma, South Carolina, Tennessee, or Texas:

You can also submit the SSA-44 form online through the Social Security Administration's website. To do so, follow these steps:
- Go to the Social Security Administration's website at www.ssa.gov.
- Click on the "Apply for Benefits" tab.
- Select "Report a Change" from the drop-down menu.
- Follow the prompts to complete the online form.
