Yale email medical records

As a Yale University student, you may have access to your medical records through the Yale Health website or the Yale Health mobile app. Here are the steps to access your medical records:

Yale Health Website:

  1. Go to the Yale Health website (www.yalehealth.yale.edu).
  2. Click on "Patient Portal" at the top right corner of the page.
  3. Log in with your Yale University NetID and password.
  4. Click on "Medical Records" from the menu.
  5. You can view your medical records, including lab results, medication lists, and appointment schedules.

Yale Health Mobile App:

  1. Download the Yale Health mobile app from the App Store (for iOS devices) or Google Play Store (for Android devices).
  2. Log in with your Yale University NetID and password.
  3. Tap on "Medical Records" from the menu.
  4. You can view your medical records, including lab results, medication lists, and appointment schedules.

Requesting Medical Records:

If you need to request a copy of your medical records, you can do so through the Yale Health website or by contacting the Yale Health Medical Records department directly. Here are the steps:

  1. Go to the Yale Health website (www.yalehealth.yale.edu).
  2. Click on "Patient Portal" at the top right corner of the page.
  3. Log in with your Yale University NetID and password.
  4. Click on "Medical Records" from the menu.
  5. Click on "Request Medical Records" and fill out the online request form.
  6. Alternatively, you can contact the Yale Health Medical Records department at (203) 432-0123 or [email protected] to request a copy of your medical records.

Note: You may need to provide identification and proof of insurance to access your medical records.
