Renew health card by mail
To renew your health card by mail, you will need to follow these steps:
Step 1: Gather Required Documents
- Your current health card (OHIP card)
- Proof of identity (e.g. driver's license, passport, or birth certificate)
- Proof of Ontario residency (e.g. utility bill, lease agreement, or bank statement)
Step 2: Fill Out the Application Form
- You can download and print the Health Card Renewal Application Form (Form 4649) from the Ontario government's website or pick one up from a ServiceOntario centre.
- Fill out the form completely and accurately, making sure to include all required information.
Step 3: Attach Required Documents
- Attach a copy of your current health card (OHIP card) to the application form.
- Attach proof of identity and proof of Ontario residency to the application form.
Step 4: Mail the Application
- Mail the completed application form, along with the required documents, to the address listed on the form.
Step 5: Wait for Your New Health Card
- Once your application is processed, you will receive a new health card in the mail.
