Renew health card by mail

To renew your health card by mail, you will need to follow these steps:

Step 1: Gather Required Documents

• Your current health card (OHIP card)
• Proof of identity (e.g. driver's license, passport, or birth certificate)
• Proof of Ontario residency (e.g. utility bill, lease agreement, or bank statement)

Step 2: Fill Out the Application Form

• You can download and print the Health Card Renewal Application Form (Form 4649) from the Ontario government's website or pick one up from a ServiceOntario centre.
• Fill out the form completely and accurately, making sure to include all required information.

Step 3: Attach Required Documents

• Attach a copy of your current health card (OHIP card) to the application form.
• Attach proof of identity and proof of Ontario residency to the application form.
